Daycare Enrollment Forms Checklist: What Providers and Parents Should Prepare

A clear enrollment packet helps a daycare provider collect consistent information and gives families a reliable place to document routines, permissions and payment expectations.

Child and guardian information

Collect the child’s preferred name, birth date, guardian contacts, authorized pickup people and reliable emergency contacts. Establish a process for verifying pickup authorization.

Health, allergy and medication information

Ask families to document allergies, dietary restrictions, relevant health information and physician contacts. Medication procedures should follow licensing rules and require the appropriate written authorization.

Daily routine and care preferences

Record meal, nap, toileting and comfort routines. A simple daily report can then communicate meals, rest, activities and supplies that need replenishment.

Permissions and policies

Use separate, clearly worded permissions where required for photographs, transportation, activities or emergency care. Families should receive current copies of attendance, illness, closure and late-pickup policies.

Tuition and payment tracking

Document rates, due dates, deposits, late-payment terms and receipts. A payment ledger should be easy for both the provider and family to understand.

Incident and communication records

Maintain consistent records for injuries, behavioral incidents, parent communications and required follow-up while protecting private information.
